
FROM January 1, 2023, Greater Shepparton will have a new provider of the Home and Community Care Program for Young People (HACC-PYP).
The outgoing provider, Nexus Primary Health, worked closely with MHA Care and the Department of Families, Fairness and Housing the Department of Families, Fairness and Housing through 2022 to ensure a smooth transition from Nexus to MHA Care.
“The goal of both service providers and DFFH was to ensure that HACC-PYP services continued to be offered and supported in the City of Greater Shepparton area. This included transitioning existing Nexus clients across to MHA Care. In recent months MHA Care has been contacting those clients and working with them to reassess their individual needs and review and update the services they receive,” said CEO of MHA Care, Michael Hogan.
MHA Care has provided HACC services in the Moira Shire for 20 years.

This program provides basic support and maintenance services to help people under 65 years of age (or under 50 for Aboriginal and Torres Strait Islander) with disability, and their unpaid carers, to remain living at home as independently as possible. The program is funded and managed by the Victorian Department of Health and Human Services.
Nexus Primary Health would like to thank the community and clients for their support over the past three years of providing services to the City of Greater Shepparton.
