NAVIGATING aged care can be overwhelming for families. That is where Community Interlink plays an important role.

With over 30 years of experience, Community Interlink specialises in ‘Support at Home’, enabling seniors to continue living independently and confidently in their own homes as they age.

Governed by GV Health, Community Interlink is a non-profit public health provider, offering benchmark expertise and quality service by its highly-qualified staff.

Community Interlink supports people to access a wide range of services through the Support at Home program, tailored to each individual’s unique needs, goals and circumstances. Services include home cleaning and gardening, transport, shopping assistance, meal preparation, delivery of pre-prepared meals, personal care, nursing and allied health services such as occupational therapy, support to stay connected with family and community, falls prevention programs and much more.

A key part of Community Interlink’s role is helping individuals and families understand and navigate the services available to them. The team works alongside service consumers to identify options, plan for changing circumstances, and ensure their choices, rights and preferences remain at the centre of every decision.

“We help people understand how they can make the most of their funding and connect them with the services that best suit their needs, goals and preferences” said Tricia.

“Our Care team takes the time to listen, understand what matters most to each person, and source the best possible support options for them.”
